Job Overview: We are seeking a compassionate and skilled Registered Nurse to join our Home Health team. As a Registered Nurse - Home Health, you will provide high-quality care to patients in the comfort of their own homes. Responsibilities: - Conduct comprehensive assessments of patients' health status - Develop and implement individualized care plans - Administer medications and treatments as prescribed - Monitor and evaluate patients' progress and update care plans accordingly - Educate patients and their families on managing their conditions and promoting wellness - Collaborate with interdisciplinary healthcare team members to coordinate patient care - Document all patient interactions, interventions, and outcomes accurately and timely
Qualifications: - Valid Registered Nurse (RN) license in the state of Illinois - Minimum of 1 year of nursing experience, preferably in home health or a related setting - Proficient in using electronic medical record systems, such as Axxess - Strong knowledge of medical office standards and procedures - Experience in geriatrics and home health - Ability to work independently and prioritize tasks effectively - Excellent communication and interpersonal skills
